Community Based Services
These services are designed to assist older individuals in avoiding institutionalization and assist individuals in long-term care institutions who are able to return to their communities, including:

Client assessment, case management services, and development and coordination of community services

In-home services and other community services, including home health, homemaker, shopping, escort, reader, and letter writing services, to assist older individuals to live independently in a home environment

Health (including mental health), education and training, welfare, informational, recreational, homemaker, counseling, or referral services

Transportation services to facilitate access to supportive services or nutrition services, and services provided by an Area Agency on Aging

Supportive activities to meet the special needs of caregivers, including caretakers who provide in-home services to frail older individuals
